Job Title: Child and Family Health Nurse – Maryborough/Hervey Bay
Practice Information:
We are Queensland’s second largest AMS (established in 2007). We provide a Culturally appropriate wrap around service to the Fraser Coast’s Indigenous communities through our two clinics in Hervey Bay and Maryborough, as well as our Outreach Service to the local surrounding areas. Delivering comprehensive primary care, promoting healthy lifestyles, supporting social and emotional wellbeing, and with an emphasis on early intervention starting with our youngest and most vulnerable community members.
Position Description:
The Child Health Nurse role is responsible for the provision of culturally appropriate maternal and child health care services to Aboriginal and Torres Strait Islander postnatal women & newborns, their children and families, within a primary health care setting. We take a holistic approach to prevention, early detection and management of child developmental problems, and the role will have a strong focus on early childhood services in accordance with our Paediatric service delivery model. Hours are negotiable, generally Monday to Friday within clinic business hours, funding dependent.
Your core responsibilities would include:
Ensuring services and administrative practices are consistently delivered in accordance with our Model of Care.
Providing effective first point of medical engagement including: triage assessments, taking medical history/detail, assignment of patient, completion of preliminary diagnostic procedures and nursing intervention.
Carry out routine screening services to patients including: immunisations, Health Checks, developmental screenings and GPMPs, Point of Care testing, hearing and eye screening, and wound management.
Manage patient referrals to wraparound services – internal and external, and the management of case conferencing.
Initiate and manage patient recall and reminder processes.
Child health services including: child development checks; screening; parenting advice; responses to identified social and developmental health needs; referral to support services; early intervention services; improved linkages and collaboration with various early childhood and allied health services.
Maternal antenatal care services including: providing advice about smoking cessation, healthy eating and physical activity; referrals to other health services, support services and specialists; parenting advice; social and emotional wellbeing; alcohol consumption during pregnancy; antenatal consultations ( if a currently registered Midwife), or working with Midwife to ensure access to antenatal services.
Postnatal care services including: Postnatal infant health checks, developmental assessments, breastfeeding support/information; parenting advice, education on providing supportive and nurturing healthy (including smoke free) environments, nutrition and healthy eating support.
Provide evidence-based assessments according to the Key Ages and Stages framework. Screening and triage for Paediatric clinics including Strength and Difficulties Questionnaires, Vanderbilt and other developmental/behavioural screening/diagnostic tools.
Child and adult immunisations.
Health assessment services: Annual Health checks, Audiometry and Tympanometry screening, Developmental milestones, Phlebotomy, and POC testing (urinalysis, Hb, HbA1c).
Health promotion services including: parenting, providing advice on nutrition, tackling smoking and alcohol, physical activity, sexual health, blood borne viruses, prevention of communicable disease; holistic health services, women’s health services, Men’s health and harm and injury reduction activities.
Liaise and coordinate treatment, care and follow-up through consultation with other sections in the service, local hospital and agencies.
Conduct home visits and/or outreach services.
Management of all recalls for significant results and follow-up of tests, and referrals.
Qualifications & Experience:
Registered Nurse with current unconditional AHPRA registration (General and/or Midwifery Nursing) and a Post Graduate Certificate in Child Health.
Proven clinic experience and sound clinical skills, and evidence of recency of practice.
Demonstrated capacity to implement and support a medical/primary health care team, and to value add through planning and coordination of your own and your team’s workload to achieve results.
Knowledge/experience in the implementation of the Ages and Stages Framework and Maternal and Child Health program standards and guidelines.
Established ability to comply with O&HS, Infection Control and Quality & Risk Management processes.
Current Endorsement for Schedule Medicines for Registered Nurses and Midwives as appropriate.
Current Cardiopulmonary Resuscitation (annual), and First Aid Training (every 3 years).
Prior experience in cultural reflective practice (or willing to attend training) and responsive to the needs of Aboriginal and Torres Strait Islander people.
Prepared to undertake a minimum 20 hours annual professional development to be eligible for ongoing Registration with (AHPRA).
A demonstrated ability to work within Aboriginal Community Controlled Health organisation.
Satisfactory Police Check.
Current Class C Drivers Licence.
Evidence of immunisation for vaccine preventable diseases
