Job Title: Nurse Practitioner - AMS – Rockhampton, Queensland.
Hours: Permanent - Full Time
Practice Information:
This Aboriginal and Torres Strait Islander community controlled primary health care service has been operating in the Rockhampton and Central Queensland region for close to 30 years. The organisation is a bulk billing community-controlled health service, managed by a Board of Directors who oversee and manage the strategic directions of the service. The service provides comprehensive primary health and well-being services to the Aboriginal and Torres Strait Islander community in the Rockhampton and surrounding regions. The practice collaborates with health and well-being partner agencies to enable integrated continuity of care for the community and continue to work to contribute to Aboriginal and Torres Strait Islander health policy and program reform in Queensland.
Position Details:
The organisation is growing and the team is growing with them. Multiple positions have come available at our Rockhampton Clinic, Blackwater and Mount Morgan for AHPRA registered Nurse Practitioners. These are permanent, full time, Monday to Friday positions (no shift work!). If you are an expert at time management, problem solving and stakeholder relations, this could be the perfect opportunity for you. In this role you will provide expert care to patients and their families in accordance with Nurse Practitioner legislation including:
Patient assessment and diagnosis
Prescribe medications, order therapies, and recommend lifestyle modifications to manage chronic conditions effectively
Ordering and analysing pathology and radiological testing
Referring patients to various Health Professionals
Participate in peer and case review procedures
Contribute to clinic workforce development programs
Providing treatment and education to patients with chronic diseases
Develop individualized care plans tailored to each patient’s unique needs, considering factors like lifestyle, comorbidities, and personal preferences
Educate patients about their conditions, treatment options, and self-management strategies.
Promoting health literacy and patient engagement and assist individuals with chronic diseases make informed decisions about their well-being.
Qualifications & Experience:
Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a nurse practitioner [or undertaking study that will lead to endorsement as a nurse practitioner by the NMBA].
Completed an NP-focused graduate master’s or doctoral nursing program.
Successfully passed a national NP board certification exam.
Demonstrated operational, financial, human resource, workplace health and safety and effective communication / interpersonal skills experience at departmental level and comprehensive understanding of clinical governance and current nursing issues.
Ability to deliver, evidenced-based, patient-centered care within a complex environment and work under a scope of practice.
Ability to conducting thorough assessments and making accurate diagnoses in chronic disease management.
Demonstrated experience in quality improvement principles, clinical policy and procedure development and implementation.
Computer literacy in MS Word/Excel, Email and an electronic medical record /referral system.
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice.
Demonstrated high-level leadership within the workplace and across external agencies including the ability to form productive working relationships through highly developed communication, negotiation, facilitation and conflict resolution skills.
Salary Information:
$120,000 – $140,000 per annum.
