Practice Information:
This practice currently provides a range of free allied health services for children and their families through our regional service Centres―in Bundaberg, Dalby, Emerald, Mount Isa, and Warwick―each staffed by a multidisciplinary team comprising Speech Pathologists, Occupational Therapists, Psychologists and Family Health Support Workers.
They also support rural communities from their four smaller satellite sites―in Inglewood, Stanthorpe, Agnes Water/Miriam Vale, and Kingaroy―staffed by Early Intervention Facilitators who can call on resources from the nearest service Centre or the Clinical Leadership Team in Brisbane.
Position Description:
The Social Worker provides evidence-based family-centred support and programs for children and families living in regional, rural and remote areas. The Social Worker is a core multidisciplinary and transdisciplinary team member, and provides a link between the therapy team and the child’s family and support network by offering comprehensive assessments, and individualised support and/or group programs to develop the capacity and knowledge needed for healthy, strong families.
This role is key to enhancing service delivery for children and their families through assessment and identification of barriers/challenges and assisting in goal setting and planning of services which are responsive and tailored to family’s needs, cultural beliefs, social and environmental circumstances.
As part of the Children’s Allied Health Team you will:
Collaborate with families and other organisations in the provision of social work services to children who require support for their Social/emotional development in accordance with this practices’ model of care.
Actively participate in the multidisciplinary team setting with particular emphasis on the delivery of social work services to optimise development and independence for children and their families.
Promote the importance of early intervention and its role in preventing long term educational and behavioural issues through community development and health promotion strategies.
Key Responsibilities:
Be responsible for assessment, planning, delivery, evaluation, and continuous improvement related to the provision of the scope of the incumbent’s discipline.
Provide care when and where it best meets the clients’ needs including on site, at home and in the community.
Provide a fly-in/out (FIFO) / drive-in/out (DIDO) service to regional Queensland areas (as required) to provide face to face support to families, the team and key stakeholders. This may involve individual assessment and intervention, seminars and/or workshops. Frequency and manner (FIFO/DIDO) is dependent on the requirements and based on the Service Centres being supported. All travel costs will be covered by the organisation.
Have a comprehensive understanding of professional governance structures and adhere to professional practice standards.
Undertake accurate and timely documentation and communication. This includes but is not exclusive to statistical information, clinical notes, reports, responses to enquiries, verbal and written documentation to referrers, clients and others.
Maintain accurate records of social work service provision to clients in accordance with medico-legal standards.
Use evidence to deliver services ensuring best practice guidelines are followed.
Contribute to quality improvement activities and participate in research.
Contribute to and participate in professional development as agreed in your Professional Achievement and Development Plan (PADP).
Staff are to keep informed about current research in early childhood intervention relevant to your role by maintaining professional knowledge and responding appropriately to unsafe or unprofessional practice, fulfilling duty of care by attending training as provided by the organisation and seeking further relevant training externally.
Integrating organisational policies and procedures in order to provide effective and professional care in a way that respects the rights and beliefs of all individuals yet maintains practice within own approved scope of practice.
Qualifications & Experience:
Tertiary qualifications in Social Work
Membership to the Australian Association of Social Workers (AASW)
Valid Blue Card – Working with Children – No Card No Start OR Combined NDIS Worker Screening Clearance and Blue Card
Current C Class Queensland Drivers Licence
Proof of Hepatitis B Vaccination status
Proof of COVID-19 vaccination or certificate stating medical contraindication
Experience with planning, implementing and evaluating small group programs with children and/or their parents/carers
Experience in working in childhood education, intervention or other relevant work experience
Salary Information:
Annual Salary - $74, 984 - $90,874 negotiable upon qualifications and experience.
