Little Haven Palliative Care is a specialist community based palliative care service operating in the Sunshine Coast area of Queensland. The service seeks to improve the ability of our community to support people facing terminal illness, grief and bereavement. It includes a specialist home care team that offers support to individuals facing terminal illness and their families supporting people’s choice to die in their own home with their loved ones around them and optimising quality of life. It also includes bereavement support and enhancing community capacity through education about palliative care, death, and dying, and loss and grief. The position of Registered Nurse / Clinical Nurse is based in the Little Haven Palliative Care office. The office is conducive to a relaxed, informal working environment for staff and volunteers. Little Haven is committed to creating a supportive, inclusive and participative working environment for staff.
Position Description:
Provision of palliative care and cancer support services, including nursing, counselling and support to palliative care clients and their families and carers, within the home of the client, or local hospitals and nursing homes. Assess client care needs, make decisions on clinical palliative care in the home, and in conjunction with the team, coordinate client care, refer and liaise with GP, medical specialists, support services according to the policies set down by Community Health and Community Health Nursing Services. Identify, limit, and report any safety and security risks, manual handling risks associated with care provided in the community and the clients home. Abide by the OH&S policies of SLHD, Community Health and Community Health Nursing. Ensure all activities and functions associated with the role address specific requirements identified in the Quality Policy and Quality Objectives. Participation in the development and delivery of an education program for volunteers, staff, palliative care clients and their families. Liaison with all other service providers within the scope of cancer, terminal and palliative care, in order to provide quality accessible care for all members of the community requiring such services. Such providers include relaxation and complementary therapy practitioners, Community Health, Department of Veteran’s Affairs, occupational and physio therapists. Utilisation of computer-based technology to maintain client database utilising the PalCare System.
Qualifications & Experience:
Registered Nurse with 4 + years palliative care and / or community nursing experience.
